Exact Match

When searching for records by Agency Code, Individual Number, Company Code, NAIC Code or Filing Number, you should enter the exact number you are looking for.

      Example:

      123456 will retrieve 123456

      123456 will not retrieve 1234 or 1234567 

Partial Match

When searching for records by Name (Agency, Individual or Company) or City, you should enter the first few letters of the Name or City you are looking for. If you are not sure of the first few letters of the Name or City, you should enter the percent sign (%) at the beginning, as explained in the example.

      Example:

      BLUE CROSS will retrieve BLUE CROSS, BLUE CROSS & BLUE SHIELD and BLUE CROSS AND BLUE SHIELD

      BLUE CROSS will not retrieve ALLIANCE BLUE CROSS & BLUE SHIELD

      You can search for the following (all Names which has BLUE CROSS in it) using %BLUE CROSS

           BLUE CROSS BLUE SHIELD

           BLUE CROSS & BLUE SHIELD

           BLUE CROSS AND BLUE SHIELD

           ALLIANCE BLUE CROSS & BLUE SHIELD

Note: The name of a Company or an Agency could be stored as abbreviations in some instances.

      Example:

      STATE FARM LIFE INSURANCE COMPANY could be stored as STATE FARM LIFE INS CO in the DOI database.

      In such cases, we suggest that you search for the company using INS%CO% in place of INSURANCE COMPANY.
