Requirements for Prelicensing Approval

1. Properly Completed Application for Prelicensing Sponsor Approval (Form SCID 3613A)

2. Properly completed Application for Instructor Approval (Form SCID 3618)

3. Appropriate examination content outline with page numbers where topic is located in course material must be submitted in duplicate. Outlines may be obtained from Experior's website at http://www.experioronline.com/scol0804.asp.

4. Prelicensing course material, quizzes and a master copy of each examination must be submitted in duplicate.

5.  A $100 filing fee (new sponsors and renewed sponsors)

6. Classroom applications must include a schedule identifying the topics that will be taught each day, time class will begin each day, time of breaks, time of lunch and time class will end each day.

7. Correspondence course applications must include details on how the individuals taking the course will be supervised. If review sessions are conducted for students, then please provide details on how these sessions will be conducted.
