                                                Personal Information Data Sheet

The purpose of this notice is to inform all resident and nonresident South Carolina producers licensed with unlimited lines of authority of their responsibility to pay a one-time $35 administrative fee to the South Carolina Department of Insurance and to complete a Personal Information Data Sheet. Unlimited lines producers are those producers licensed with one or more of the following lines of authority: life, accident and health, property, casualty, surety, marine. All producers licensed on January 31, 2003 with unlimited lines of authority are required by law to pay this fee (See Section 38-43-80). The fee will be used to cover the cost of implementing the requirements of the new producer model law. Pursuant to South Carolina Code Ann. Section 38-43-80, failure to pay the $35 fee by 5:00 pm, April 15, 2003 will result in the producer’s license being canceled. In order to regain licensing privileges, the producer will be required to pay a penalty of $250.

The following items must be returned to the Department of Insurance by 5:00 p.m., April 15, 2003:

  1. A $35.00 administrative fee paid by the producer. However, if an insurance agency is paying the $35 fee on behalf of its producers, the agency must submit the Personal Information Data Sheet for all producers of the agency, and attach one check or money order for payment for all producers.

  2. A properly completed Personal Information Data Sheet. All blank spaces must be completed. If any preprinted information on the data sheet that you receive is incorrect, i.e., mailing/residence address, name spelling, etc., please make the appropriate correction(s) on the data sheet.

  3. A self-addressed, stamped envelope. A new license will be mailed to you in the envelope provided.  A Producer can check his/her licensing status on the Department’s website at: www.doi.sc.gov

The Department must receive all listed items mentioned above before a new license is issued. An interim license is enclosed, but will expire at 5:00 pm April 15, 2003. If the producer has not complied with the above requirements by April 15, 2003, no functions of a licensed producer may be performed after that date. Insurers are prohibited by law from accepting policies, paying commission, or permitting an unlicensed producer to perform any of the requirements of a licensed producer on any policy of insurance written by them.
